Applications will not be accepted without all the documents listed below:
- Summer Camp Application (all areas completed)
- Attendance Sheet
- Immunization Record
- Medical Authorization/Allergy (if applicable)
- Behavior Policy
- Parent/Camper Contract
- Field Trip Permission Form
- Change Request Form (keep form)
- One-on-One Assistance Policy (must be signed)
