Parent/Camper Contract

TANF Eligibility Form:

As part of our summer camp funding, families may qualify for additional support through the Temporary Assistance for Needy Families (TANF) program.

If you believe your child may be eligible based on household income, please complete the TANF Services Certification Form and return it with your application.

Completion of this form is optional but may help determine eligibility for financial assistance and program funding.
