- Summer Camp Application (all areas completed)
- Attendance Sheet
- Immunization Record (up to date medications)
- Medical Authorization/Allergy (if applicable)
- Behavior Policy
- Parent/Camper Contract
- Field Trip Permission Form
- Change Request Form (keep form)
